County Code section 17-410 and Virginia Code §62.1-44.15:34 requires the VSMP authority to
act on any VSMP permit by issuing a project approval or denial. This project is denied. The
rationale is given in the comments below. The application may be resubmitted for approval if all
of the items below are satisfactorily addressed. The VSMP application content requirements can
be found in County Code section 17-401.
A. Stormwater Pollution Prevention Plan (SWPPP)
The SWPPP content requirements can be found in County Code section 17-405. A SWPPP must
contain (1) a PPP, (2) an ESCP, (3) a SWMP, and (4) any TMDL measures necessary.
1. Please provide a signed copy of the certification (Section 9). Rev. 1. Comment addressed.
2. Please note that the notice of general permit coverage (Section 2) and the general permit copy
(Section 11) must be inserted. Rev. 1. Comment addressed.
3. Update any plans sheets which are revised in the next submission. Rev. 1 Comment addressed.
B. Pollution Prevention Plan (PPP)
The PPP content requirements can be found in County Code section 17-404.
1. Approved, but update plan sheet with any E&S revisions. Rev. 1 Comment addressed.
C. Stormwater Management Plan (SWMP)
VSMP Regulation 9VAC25-870-108 requires the VSMP authority to approve or disapprove a
SWMP. This plan is disapproved, and the reasons are provided in the comments below. The
stormwater management plan content requirements can be found in County Code section 17-403.
1. Please combine the SWMP & ESCP into one plan set. It should be called VSMP Submittal and
have the WPO number (201600064) on it. Rev. 1 Comment addressed.
2. Provide calculations for the sizing of the 8" roof drain as well as permanent outlet protection to
prevent erosion. Rev. 1 Comment Addressed.
3. The forest/open space used for VRRM calculations will need to be platted before this plan can be

approved. Please note that the County Attorney is currently working on a template for a deed to go
with this plat. Rev. 1 Required deed and agreement are provided with this letter. The forest
and open space should be labeled as "SWM forest & open space easement".
4. Please clarify the total areas and types of cover within the drainage divides. The charts on sheets 6,
7 and 9 all have different numbers. Rev. 1 Comment Addressed.
5. There are sheet flow labels on Sheet 8 that don't appear to be pointing towards any Tc path.
Please remove or clarify. Also, there are no Tc paths shown for Drainage Area A. Please provide.

D. Erosion and Sediment Control Plan (ESCP)
Virginia Code §62.1-44.15:55 requires the VESCP authority to approve or disapprove an ESCP.
This plan is disapproved, and the reasons are provided in the comments below. The erosion control
plan content requirements can be found in County Code section 17-402.
1. The diversion dike on the western end of the site may be changed to silt fence. All diversion dikes
need to be directed to sediment traps or basins. There is not much drainage on this end of the site
and in this case a trap or basin is not necessary. Silt fence will be sufficient. This silt fence should
be included in Phases I and II.
2. Please show silt fence on the eastern end of the site adjacent to the staging and stockpile areas.
This silt fence should be included in Phases I and II.
3. Please show silt fence as well as tree protection on the southern perimeter of the site in Phase II.
The VSMP permit application and all plans may be resubmitted for approval when all comments have
been satisfactorily addressed. For re -submittals please provide 2 copies of the complete permit package
with a completed application form.
Engineering plan review staff are available from 2-4 PM on Thursdays, should you require a meeting to
discuss this review.
Process;
After approval, plans will need to be bonded. The bonding process is begun by submitting a bond estimate
request form and fee to the Department of Community Development. One of the plan reviewers will
prepare estimates and check parcel and easement information based on the approved plans. The County's
Management Analyst will prepare bond agreement forms, which will need to be completed by the owner
and submitted along with cash, certificates or sureties for the amounts specified. The agreements will need
to be approved and signed by the County Attorney and County Engineer. This may take 2-4 weeks to
obtain all the correct signatures and forms.
Stormwater Management Facilities Maintenance agreements will also need to be completed and recorded.
The County's Management Analyst or other staff will prepare the forms and check for ownership and
signature information. The completed forms will need to be submitted along with court recording fees.
After bonding and agreements are complete, county staff will need to enter project information in a DEQ
database for state application processing. DEQ will review the application information based on local
VSMP authority approval. At this time, the DEQ portion of the application fees will need to be paid
directly to the state. For fastest processing, this is done electronically with the emails provided on the
application. DEQ should notify applicants with instructions on how to pay fees. When DEQ approves the

application, they will issue a permit coverage letter. This should be copied to the county.
After DEQ coverage is issued, via the coverage letter, the County can hold a pre -construction conference.
Applicants will need to complete the request for a pre -construction conference form, and pay the remainder
of the application fee. The form identifies the contractor and responsible land disturber, and the fee
remaining to be paid. This will be checked by county staff, and upon approval, a pre -construction
conference will be scheduled with the County inspector. At the pre -construction conference, should
everything proceed satisfactorily, a joint VSMP and grading permit will be issued by the County so that
work may begin.

Response: A plat was prepared to delineate the forest/open space used

preliminary copy of the deed document has been completed and provided
with the plat.

Response:
. -&Response: The VRRM spreadsheet on sheet 7 evaluates the water quality
requirements for the site. Therefore, only the on-site areas are included.
The proposed drainage divides tabulation on sheet 9 lists the proposed
drainage areas which include drainage from Hacktown Road that flows
y/ on-site. Therefore, the drainage from the off-site areas is included for
/ water quantity evaluation purposes. The tabulation on sheet 6 lists the
��V subareas within the overall drainage in order to evaluate each segment of
L the drainage path. The roof drainage area is shown to evaluate the roof
drain, the upstream area of the driveway culvert is shown to evaluate it
and the total area to each level spreader is shown on sheet 6. The
computations use the rational method and profiles/sections are shown on
sheet 5. Please note that the area breakdown and total for each level
spreader(LS#1 and LS#2) on sheet 6 is the same as DA B1 and DA B2 on
sheet 9.

Response: The erroneous text was deleted. The Tc for Drainage Area 'A'
/ in both the pre and post condition was determined to be sheet flow with
IL less than the minimum of 0.1 hr or 6 min. Therefore, the Tc path was not
shown. The Tc is listed in the Drainage Area tabulation for both the
existing and proposed drainage divides. See sheets 8 and 9.

trap was deleted. The silt trap computation was deleted. The silt fence is
shown in both Phase I and Phase II. References to the diversion dike and
silt trap were deleted from the Erosion and Sediment Control Legend for
Phase I and Phase II. See sheets 16 and 17. The silt trap and diversion
dike details were deleted from the Erosion and Sediment Control Details.
0 See sheet 19. Reference of the diversion dike and silt trap were deleted
from the list of structural practices on the Erosion and Sediment Control
Notes. The Erosion and Sediment Control Escrow estimate was updated
to include the additional silt fence, tree protection, outlet protection and
delete the diversion dike and silt trap. See sheet 18.
