- Owner's signature and date of signature on the application form (17-401-A).
- Registration Statement (17-401.C)
- Contact information for the owner — phone number and email (17-403.A.2).
- Description of long-term maintenance operation and maintenance (17-403.A.5).
- Documentation demonstrating compliance (e.g. Routing Calculations) (17-403.A.7).
- Offsite compliance options — provide letter of availability (17-403.A.9).
- Item F on the form —the Pollution Prevention Plan (17-401.F and 404).
- Item G on the form — the Stormwater Pollution Prevention Plan (17-401.G and 405).
- These items also need to be checked off on the form as being submitted.
If this information is not received by May 151h, the application will be rejected and a new application and
fee shall be required.
The application shall not be accepted for review until it is determined to be complete.
