Box 359, Keene, VA 22946 [lkuttnerna,hampshireinvest.com ] Plan received date: 5 April 2018 Date of comments: 23 May 2018 Reviewer: John Anderson County Code section 17-410 and Virginia Code §62.1-44.15:34 requires the VSMP authority to act on any VSMP permit by issuing a project approval or denial. This project is denied. The rationale is given in comments below. The application may be resubmitted for approval if all of the items below are satisfactorily addressed. The VSMP application content requirements can be found in County Code section 17-401. General 1. Provide a stand-alone VSMP plan set, including sheets: a. Sheets 1-10, 16-20 only. b. Sheet 1: Title plan set VSMP Plan /WPO #201800026 for Caliber Collision c. Sheet 8: for VSMP Plan, re -title this sheet Site Layout 2. Sheets 2, 4, 5, 8, 16: a. Provide additional contour labels; b. Show /shade preserved steep slopes; c. Show FEMA floodplain boundary; d. Approximate existing tree line. 3. Provide SWPPP (booklet) using county template. (See A., below.) 4. Provide /record permanent drainage easement extending from SWM facility easement, to include detention pond outfall pipes through Str. 9, and continuing from Str. 9 to Moores Creek, prior to VSMP approval. 5. Provide SWM facility access and maintenance easement (deed /plat), and record SWM Maintenance Agreement for dry pond (with access extending to Avon Street Extended), prior to VSMP approval. 6. Septic Field design requires VDH approval. Albemarle County does not coordinate VDH approval. Applicant is urged to coordinate septic design review and approval with VDH as soon as possible. 7. Albemarle County Engineering Division does not review, approve or warranty private landscape irrigation systems, though the system may be reviewed with building permit application. 8. Adopt VDOT nomenclature, for example DI-2A, instead of double combination inlet, if DI-2A intended (sheet 18). 9. Title sheet —revise Vicinity Map A. Stormwater Pollution Prevention Plan (SWPPP) The SWPPP content requirements can be found in County Code section 17-405. A SWPPP must contain (1) a PPP, (2) an ESCP, (3) a SWMP, and (4) any TMDL measures necessary. Exhibit should indicate reasonable locations for: solid waste receptacle, on -site fuel, porta-potty, concrete washout, etc. Contractor may later revise the Exhibit. For on - site fuel storage, provide containment sufficient to impound the volume of fuel stored + 10-yr rainfall event. SWM Plan 1. Water Quality: Please continue to coordinate with Ana Kilmer, Albemarle County on purchase document requirements for county approval of nutrient credits to satisfy VaRRM spreadsheet calculation that 1.38 lb/yr phosphorus reduction is required for this project, based upon site land cover conditions. Ana will check form of documents, as well as availability /eligibility of proposed bank to provide required credits. 2. Water Quantity: Revise and re -run routings (if necessary) using correct Device #1 pipe outfall diameter (24") and length (179'). This should not affect review approval, but it is important that routings reflect proposed design. 3. Note: Routings have been discussed and small -diameter orifice size (<3") eliminated as a design option, at County direction. Small diameter orifices have a tendency to block and short-circuit detention performance. Routings are acceptable to Albemarle and the county supports detention pond design relative to water quantity pre- /post development 1-yr, 24 hour storm event release rates, relative to channel protection. 4. Sheet 17: a. Section, Detention Pond Outlet Structure Detail: i. Bottom of structure El = 462.5 conflicts with note to extend riser f 12" below bottom of pond; revise El. ii. Revise label to read `extend riser f 12" below bottom of outlet barrel.' iii. Clarify bottom of skimmer plate El=466.75 (please call to discuss). iv. Provide elevation label, bottom of concrete base. v. Confirm 5' X 5' concrete riser is square. Recommend revise label to clarify. b. Provide dry pond plan view (sheet 5 view would work). c. Label floor dimensions of dry pond (plan view) to aid inspection. d. Pond Section detail does not include 3' sump; revise to show 3' sump. 5. Sheet 18: a. Line 1, 2: Revise F.E.S. to read Flared End Section, if flared end section specified. b. Show riprap (outfall protection) in storm drain profiles. c. Storm Drain Line 1: Recommend VDOT nomenclature; DI-2A, for example. 6. Sheet 19 a. Trash Rack Detail: Recommend show anti -vortex device dimensions, or provide note /description of anti -vortex device with this detail. b. Trash Rack Detail: Provide and show manway access via trash rack to detention pond riser, for periodic maintenance inspection. C. Energy Dissipator Detail: Specify outfall protection riprap stone size. d. Energy Dissipator Detail: Confirm 12" PVC pipe sleeve drain diameter is accurate. B. Pollution Prevention Plan (PPP) The PPP content requirements can be found in County Code section 17-404. 1. See A., above. County template includes suggested PPP (Sec. 6). C. Erosion and Sediment Control Plan (ESCP) Virginia Code §62.1-44.15:55 requires the VESCP authority to approve or disapprove an ESCP. This plan is disapproved, and the reasons are provided in the comments below. The erosion control plan content requirements can be found in County Code section 17-402. 2. 3. 4. 5. Sequence of Construction, Phase II, Note 13: Revise to require inspections at least once every four (4) business days, or at least once every five (5) business days and no later than 48 hours following a measurable storm event (>0.25" rainfall; on -site rain gauge). —ref. VPDES CGP VAR10, Part 1 /B.4.(d)l. d. The applicable SWPPP inspection requirements specified in Part II F 2 shall be amended as follows: (1) Inspections shall be conducted at a frequency of (i) at least once every four business days or (ii) at least once every five business days and no later than 48 hours following a measurable storm event. In the event that a measurable storm event occurs when there are more than 48 hours between business days, the inspection shall be conducted on the next business day; and b. Eliminate keynotes O, W. These curb types do not appear to match VDOT standards. e. Label and provide dimension for walkway fronting Avon Street Ext., if walkway proposed. f. Provide Note that dry pond will be posted: `No Trespassing; drowning hazard,' since sump depth during 10-yr event may approach 6.46'. g. Provide several entrance width dimensions that can easily be correlated to sidewalk, face of island, west /interior end of island, etc. h. Provide (with Final Site Plan) VDOT GC-12 label for ramp N of entrance. i. Show SWM facility maintenance, access, and drainage easements. 6. Sheet 9 a. Recommend Wood Privacy Fence detail show posts set in continuous CIP wall. Engineering Review Comments Page 4 of 4 b. Sidewalk Ramp Detail: Include detectable surface. c. Wall Flume Detail: provide flume outfall protection detail. Dimension riprap /provide 2-year velocity calculations for concrete flume. d. Recommend consider (flume) expansion joint spacing of less than every ninety (90) feet. e. Handrail Detail: some text is unreadable. Provide legible detail. 7. Sheet 10: Delete Type `A' C&G and `G' curb details. Use VDOT standards. Provide VDOT CG-7 detail. 8. Sheet 16: a. Provide asphalt /concrete curb upslope of dumpster enclosure to prevent cross -lot drainage entering enclosure and transporting contaminants. With curb design, ensure positive drainage to limit trapped or standing water. b. Show 4' wall flume outfall protection (riprap; 2-year velocity). As discussed 5/22, FK Inc. is welcome to submit .PDF VSMP plan set reflecting revisions. Engineering, several days past due with initial VSMP review comments, will review /comment on .PDF within 2 weeks of re -submittal, prior to print copy resubmittal. FK Inc. is welcome to submit a print copy rather than .PDF. Engineering plan review staff is also available from 2-4 PM on Thursdays, should you require a meeting to discuss this review. Process After approval, plans will need to be bonded. The bonding process is begun by submitting a bond estimate request form and fee to the Department of Community Development. One of the plan reviewers will prepare estimates and check parcel and easement information based on the approved plans. The County's Management Analyst will prepare bond agreement forms, which will need to be completed by the owner and submitted along with cash, certificates or sureties for the amounts specified. The agreements will need to be approved and signed by the County Attorney and County Engineer. This may take 2-4 weeks to obtain all the correct signatures and forms. Stormwater Management Facilities Maintenance agreements will also need to be completed and recorded. The County's Management Analyst or other staff will prepare the forms and check for ownership and signature information. The completed forms will need to be submitted along with court recording fees. After bonding and agreements are complete, county staff will need to enter project information in a DEQ database for state application processing. DEQ will review the application information based on local VSMP authority approval. At this time, the DEQ portion of the application fees will need to be paid directly to the state. For fastest processing, this is done electronically with the emails provided on the application. DEQ should notify applicants with instructions on how to pay fees. When DEQ approves the application, they will issue a permit coverage letter. This should be copied to the county. After DEQ coverage is issued, via the coverage letter, the County can hold a pre -construction conference. Applicants will need to complete the request for a pre -construction conference form, and pay the remainder of the application fee. The form identifies the contractor and responsible land disturbed, and the fee remaining to be paid. This will be checked by county staff, and upon approval, a pre -construction conference will be scheduled with the County inspector. At the pre -construction conference, should everything proceed satisfactorily, a joint VSMP and grading permit will be issued by the County so that work may begin.
