County Code section 17-410 and Virginia Code §62.1-44.15:34 requires the VSMP authority to act on any
VSMP permit by issuing a project approval or denial. This project is denied. The rationale is given in the
comments below. The application may be resubmitted for approval if all of the items below are
satisfactorily addressed. The VSMP application content requirements can be found in County Code section
17-401.
A. Stormwater Pollution Prevention Plan (SWPPP)
The SWPPP content requirements can be found in County Code section 17-405. A SWPPP must contain
(1) a PPP, (2) an ESCP, (3) a SWMP, and (4) any TMDL measures necessary.
Please provide updated location of VAR100043 SWPPP. Confirm SWPPP is available for inspection. Confirm
VAR100043 includes acreage disturbed with this plan (block 7, phase 2). Most recent VDEQ permit activity
appears to be for blocks 10 and 16-18; Application Approved date, 2/8/18; Est. Area to be disturbed (Ac.) =113.73.
Please provide an updated overall development VPDES Map for Old Trail Village showing block 7, phase 2 is
covered by current VPDES permit VA100043.
SWM Plan
1. Sheet 7: Please revise Old Trail Village Water Quality Compliance Tracking Ledger to ref. Block 7, Phase
2 submittal, rather than Block 3113 & C SWM.
2. Provide copy of recordation of any required (temporary /permanent) drainage easement, prior to VSMP
plan approval.
B. Pollution Prevention Plan (PPP)
The PPP content requirements can be found in County Code section 17-404.
1. See A., above.
C. Erosion and Sediment Control Plan (ESCP)
Virginia Code §62.1-44.15:55 requires the VESCP authority to approve or disapprove an ESCP. This plan
is disapproved, and the reasons are provided in the comments below. The erosion control plan content
requirements can be found in County Code section 17-402.
Sheet 3:
1. Show contour 682.5' contour upslope of ST -1.
2. Include Note that ST -1 will not be removed or replaced with 1% stabilized channel until all upslope areas
are stabilized. 1% channel may not convey sediment -laden runoff to Upper Ballard Pond.
3. Label ST -1 floor dimensions to aid construction and inspection (41' X 91').

4. Provide or maintain IP (installed with Block 7, Phase 1) at storm inlet structures Ex. 203 (A, B), 211 (A,
B), Ex. 215, and Ex. 223 —show inlet protection on plan sheet.
5. Sheet 4: Revise `channel at l%' label to include "V -ditch"; reference ditch section detail on sheet 7.
6. Sheet 4: Channel at 1% is SWM conveyance until final build -out and conveyance system connection with
pond, via Ex. Str. 226. Provide non-erosive conveyance to lake edge (lined grass channel), or provide
energy dissipation (level spreader, etc.) to prevent sediment transport streamward of riprap shown at end of
channel. A small sediment trap downstream of riprap (shown on plan) may also work. Goal: prevent
sediment reaching Upper Ballard Pond, through final build -out.
7. Sheet 7: Compare road plan sheet 8 storm sewer computations, Str. 7, channel @ 1% sta. 12+87 (4.7 fps)
with VESCH Std. & Spec. 3.36. Road plan discharge velocity at this point appears to require EC -3, Soil
Stabilization Blanket Treatment 2. Confirm velocity and stabilization blanket design.
Revisions are slight. Engineering welcomes RGA .PDF email plan submittal (preview) prior to print re -submittal.
Engineering plan review staff are available from 2-4 PM on Thursdays, should you require a meeting to discuss
this review.
Process
After approval, plans will need to be bonded. The bonding process is begun by submitting a bond estimate request
form and fee to the Department of Community Development. One of the plan reviewers will prepare estimates and
check parcel and easement information based on the approved plans. The County's Management Analyst will
prepare bond agreement forms, which will need to be completed by the owner and submitted along with cash,
certificates or sureties for the amounts specified. The agreements will need to be approved and signed by the
County Attorney and County Engineer. This may take 2-4 weeks to obtain all the correct signatures and forms.
Stormwater Management Facilities Maintenance agreements will also need to be completed and recorded. The
County's Management Analyst or other staff will prepare the forms and check for ownership and signature
information. The completed forms will need to be submitted along with court recording fees.
After bonding and agreements are complete, county staff will need to enter project information in a DEQ
database for state application processing. DEQ will review the application information based on local VSMP
authority approval. At this time, the DEQ portion of the application fees will need to be paid directly to the state.
For fastest processing, this is done electronically with the emails provided on the application. DEQ should notify
applicants with instructions on how to pay fees. When DEQ approves the application, they will issue a permit
coverage letter. This should be copied to the county.
After DEQ coverage is issued, via the coverage letter, the County can hold a pre -construction conference.
Applicants will need to complete the request for a pre -construction conference form, and pay the remainder of the
application fee. The form identifies the contractor and responsible land disturbed, and the fee remaining to be
paid. This will be checked by county staff, and upon approval, a pre -construction conference will be scheduled
with the County inspector. At the pre -construction conference, should everything proceed satisfactorily, a joint
VSMP and grading permit will be issued by the County so that work may begin.

this proposed development.
2. Ensure the latest, approved sheets get inserted into the master SWPPP for Old Trail.

C. Stormwater Management Plan (SWMP)
VSMP Regulation 9VAC25-870-108 requires the VSMP authority to approve or disapprove a
SWMP. This plan is disapproved, and the reasons are provided in the comments below. The
stormwater management plan content requirements can be found in County Code section 17-403.
1. Please change the title on the cover sheet to be VSMP Plan and ensure the appropriate fees are
paid. This plan needs to show that it is meeting the SWM requirements, which is part of the
VSMP. Please refer to section 17-303, if under 10,000 sf and part of a common plan, it is still a
VSMP and must show compliance with the VSMP regulations.
2. Submit drainage area map with location of existing SWM facility treating site runoff from Block 7.
3. Verify that water quality/quantity compliance is accounted for with the OTV Master Plan & SWM
Quality Tracking spreadsheet.
4. Update the stormwater note on Sheet 5 to show the approved plan # for the SWM facility that is

treating this site.
5. Extend proposed storm easement along the entire proposed channel. This easement will need to be
recorded before this plan can be approved.
D. Erosion and Sediment Control Plan (ESCP)
Virginia Code §62.1-44.15:55 requires the VESCP authority to approve or disapprove an ESCP.
This plan is disapproved, and the reasons are provided in the comments below. The erosion
control plan content requirements can be found in County Code section 17-402.
1. Update the existing conditions sheet to only show what is built or to reference an approved plan
for what is shown.
2. Ensure the DB and PG is shown for all existing easements.
3. Ensure all critical slopes are shown.
4. Provide an E&S Legend.
5. Provide design calculations and detail for the proposed channel. Also, please make the side slopes 3:1, not
2:1.
6. Ensure dust control, temporary seeding and permanent seeding are shown.
7. Ensure water from diversion dikes is directed into the sediment traps. (Angle the diversion dike towards the
trap, not parallel to it).
8. Show the drainage areas to the silt fence along with the total length of the silt fence to show compliance
with the 1/4 Ac per 100 ft requirement.
9. Provide mitigation plan for the proposed buffer disturbance.
10. Provide CE in Phase II.
11. Provide detail for check dams.
The VSMP permit application and all plans may be resubmitted for approval when all comments have
been satisfactorily addressed. For re -submittals please provide 2 copies of the complete permit package
with a completed application form.
