Dear Rebecca,
Please consider this letter and revised Site Plan Sheets 1,4,and 5 as our request for a letter of revision to the referenced site
plan. The updated building footprint matches the architectural plans by Bob Anderson. The changes include the following:
1) The lead walk on the northern side of the building was shifted so that it is in front of the portico.
2) The lead walk on the southern side of the building was reduced from 6'to 5'.
3) Wheel stops were added to the upper parking lot so that the 5'sidewalk is ministerial.
4) The ADA space was shifted so that the ADA access would be the shortest path for both entrances.
5) A handrail was added to the ADA access for the southeast entrance to divert pedestrians around high step.

MAINTAINING THE EXISTING UTILITIES SHALL BE APPROVED BY THE RESPECTIVE UTILITY COMPANY
AND/OR THE ENGINEER. CONTRACTOR SHALL EXERCISE- CARE, TO INSURE THAT THE GRADE .AND
ALIGNMENT OF EXISTING UTILITIES ARE MAINTAINED AND THAT NO JOINTS OR CONNECTIONS ARE
DISPLACED. BACKFILL SHALL BE CAREFULLY PLACED AND COMPACTED TO PREVENT FUTURE DAMAGE OR
SETTLEMENT TO EXISTING UTILITIES. ANY UTILITIES REMOVED AS PART OF THE WORK, AND NOT

DRAINAGE SYSTEMS
1. CONTRACTOR SHALL EXERCISE CARE, ESPECIALLY AT INTERSECTIONS AND GUTTER LINES, TO PROVIDE
POSITIVE DRAINAGE. ANY AREAS WHERE WATER IS IMPOUNDED SHALL BE CORRECTED BY CONTRACTOR
AT NO ADDITIONAL COST. POSITIVE DRAINAGE OF ALL ROADWAY AREAS TO. THE STORM DRAIN INLETS OR
OTHER ACCEPTABLE DRAINAGE CHANNELS AS NOTED ON THE PLANS IS REQUIRED.
2. CONTRACTOR SHALL MAINTAIN EXISTING STREAMS, DITCHES, DRAINAGE STRUCTURES, CULVERTS AND
FLOWS AT ALL TIMES DURING THE WORK. CONTRACTOR SHALL PAY FOR ALL PERSONAL INJURY AND
PROPERTY DAMAGE WHICH MAY OCCUR AS A RESULT OF FAILING TO MAINTAIN ADEQUATE DRAINAGE.
3. ALL PIPES, DI'S AND OTHER STRUCTURES SHALL BE INSPECTED BY THE PROJECT MANAGER BEFORE
BEING BACKFILLED OR BURIED. THE PROJECT MANAGER MAY REQUIRE CONTRACTOR, AT NO ADDITIONAL
COST, TO UNCOVER AND RE-COVER SUCH STRUCTURES IF THEY HAVE BEEN BACKFILLED OR BURIED
WITHOUT SUCH INSPECTION.
4. ALL CATCH BASINS ENCOMPASSED WITHIN NEW CONSTRUCTION. SHALL BE CONVERTED TO DROP INLETS.
5. REMOVED PIPE SHALL BE THE PROPERTY OF CONTRACTOR . UNLESS NEGOTIATED OTHERWISE BEFORE
PROJECT BIDDING; AND IF NOT SALVAGED FOR RE -USE, SH,ALL.BE DISPOSED OF LAWFULLY.
6. ALL STORM SEWER PIPE AND DROP INLETS SHALL BE CLEARED OF DEBRIS AND ERODED MATERIAL
PRIOR TO FINAL ACCEPTANCE.
7. ALL STORM SEWER PIPE JOINTS SHALL BE SEATED AND SEALED IN ACCORDANCE WITH THE
MANUFACTURER'S SPECIFICATIONS.
8. ALL EXISTING ROOF DRAINS AND OTHER DRAINAGE CONDUIT BLOCKED OR DISRUPTED FROM THEIR
PRE -CONSTRUCTION DRAINAGE PATTERNS SHALL BE SHORTENED,, EXTENDED OR OTHERWISE ROUTED

SPECIFICATIONS. REMOVAL SHALL BE DONE IN SUCH A MANNER AS TO NOT TEAR, BULGE OR DISPLACE
ADJACENT PAVEMENT EDGES SHALL BE CLEAN AND VERTICAL, ALL CUTS SHALL BE PARALLEL OR

7. DISPOSAL OF ALL EXCESS MATERIAL IS THE RESPONSIBILITY OF CONTRACTOR.
8. SEAL (TACK) COAT SHALL BE APPLIED IMMEDIATELY BEFORE ASPHALT IS LAID AND SHALL EXTEND

ACSA GENE WATER & SEWER CONDITIONS
1. WORK: SHALL BE SUBJECT TO INSPECTION BY ALBEMARLE COUNTY SERVICE AUTHORITY (ACSA)
INSPECTORS. THE CONTRACTOR WILL BE RESPONSIBLE FOR NOTIFYING THE PROPER ACSA OFFICIALS
AT THE START OF THE WORK.
2. THE LOCATION OF EXISTING UTILITIES ACROSS THE LINE OF THE PROPOSED WORK ARE NOT
NECESSARILY SHOWN ON THE PLANS AND WERE SHOWN ARE ONLY APPROXIMATELY CORRECT THE
CONTRACTOR SHALL, ON HIS OWN INITIATIVE, LOCATE ALL UNDERGROUND LINES AND STRUCTURES, AS
NECESSARY.
3. ALL MATERIALS AND CONSTRUCTION SHALL COMPLY WITH THE CURRENT EDITION OF THE GENERAL
WATER AND SEWER CONSTRUCTION SPECIFICATIONS, AS ADOPTED BY THE ACSA.
4. DATUM FOR ALL ELEVATIONS SHOWN IN NATIONAL GEODETIC SURVEY,
5. THE CONTRACTOR SHALL BE RESPONSIBLE FOR NOTIFYING MISS UTILITY (1-800-552-7001).
6. ALL WATER AND SEWER PIPES SHALL HAVE A MINIMUM OF THREE (3) FEET OF COVER MEASURED
FROM THE TOP OF PIPE, OVER THE CENTERLINE OF PIPE. THIS INCLUDES ALL FIRE HYDRANT LINES,
SERVICE LATERALS AND WATER LINES, ETC.
7. ALL WATER AND SEWER APPURTENANCES. ARE TO BE LOCATED OUTSIDE OF ROADSIDE DITCHES.
8. VALVES ON DEADEND LINES SHALL. BE RODDED TO PROVIDE ADEQUATE RESTRAINT FOR THE VALVE
DURING A FUTURE EXTENSION OF THE UNE.
9. TREES ARE NOT PERMITTED IN THE ACSA EASEMENT.

EROSION CONTROL & WORK AREA PROTECTION AND MAINTENANCE
1. ALL FENCES REQUIRED TO BE REMOVED OR DISTURBED BY CONSTRUCTION SHALL BE SALVAGED, STORED,
PROTECTED AND RE -INSTALLED BY CONTRACTOR AT THE DIRECTION OF AND LOCATION DESIGNATED BY
THE PROJECT MANAGER. IF SUCH FENCE MATERIAL CANNOT BE REUSED DUE TO DAMAGE CAUSED BY
CONTRACTOR, CONTRACTOR SHALL INSTALL NEW FENCE OF THE SAME TYPE OF MATERIAL TEMPORARY
FENCING REQUIRED BY PRIVATE PROPERTY O.WNEIRS SHALL BE PROVIDED BY CONTRACTOR. CONTRACTOR
IS ADVISED TO CONTACT PROPERTY OWNERS AT LEAST FORTY-EIGHT (48) HOURS I IN ADVANCE OF
REMOVING ANY FENCE IN ORDER TO COORDINATE RELOCATION AND/OR PROTECTION OF ANY ANIMALS,
AND TO ESTABLISH AND CONFIRM WITH THE OWNER THE PRE -CONSTRUCTION CONDITION OF ANY FENCE
TO. BE REMOVED, DISTURBED OR REPLACED.
2. CONTRACTOR IS PERMITTED TO WORK IN THE PUBLIC RIGHT-OF-WAY (FOLLOWING ISSUANCE OF VDOT
APPROVAL) AND ANY TEMPORARY OR PERMANENT EASEMENT (FOLLOWING VERIFICATION OF EXISTENCE
OF EASEMENT) SHOWN ON. THE PLANS. HOWEVER, CONTRACTOR SHALL NOTIFY PROPERTY OWNER(S)
FORTY-EIGHT (48) HOURS PRIOR TO WORKING ON ANY PRIVATE PROPERTY TO GAIN AND COORDINATE
ACCESS AND TO DETERMINE A STORAGE AREA FOR MATERIALS IF NEEDED. COORDINATION OF ACCESS TO
PUBLIC RIGHT-OF-WAY AND, STORAGE OF MATERIALS THEREON SHALL BE COORDINATED WITH VDOT AND
THE PROJECT MANAGER. CONTRACTOR'S FAILURE TO SO NOTIFY AND COORDINATE WITH, P . ROPERTY
OWNERS AND/OR THE PROJECT MANAGER MAY RESULT IN DELAYS.
3. CONTRACTOR SHALL, AT HIS EXPENSE, MAINTAIN THE WORK SITE IN A CLEAN AND ORDERLY
APPEARANCE AT ALL TIMES. ALL DEBRIS AND SURPLUS MATERIAL COLLECTED SHALL BE DISPOSED OF
OFF THE WORK SITE BY CONTRACTOR, AT HIS EXPENSE.
4. EXISTING LAWNS, TREES, SHRUBS, FENCES, UTILITIES, CULVERTS, WALLS, WALKS, DRIVEWAYS, POLES,
SIGNS, RIGHT-OF-WAY MONUMENTS, MAILBOXES AND THE LIKE SHALL BE PROTECTED FROM DAMAGE
DURING THE WORK. ANY DAMAGE CAUSED TO SUICH ITEMS SHALL BE REPAIRED OR REPLACED BY
CONTRACTOR AT NO ADDITIONAL COST. PROPERTY PINS DISTURBED BY CONTRACTOR THAT ARE NOT
SHOWN ON THE PLANS TO BE DISTURBED SHALL BE RESTORED BY A LICENSED SURVEYOR AT
CONTRACTOR'S EXPENSE.
5. CONTRACTOR SHALL EMPLOY EROSION CONTROL DEVICES AND METHODS AS REQUIRED TO MET THE
REQUIREMENTS AND INTENT OF THE LOCAL EROSION CONTROL ORDINANCE. CONTRACTOR SHALL PROVIDE
THE NECESSARY DIVERSION DITCHES, DIKES OR TEMPORARY CULVERTS REQUIRED TO PREVENT MUD AND
DEBRIS FROM BEING WASHED ONTO THE STREETS OR PROPERTY. CONTRACTOR'S VEHICLES SHALL BE
KEPT CLEAN TO PREVENT MUD OR DUST FROM BEING DEPOSITED ON STREETS. No AREA SHALL BE LEFT
DENUDED FOR MORE THAN SEVEN (7) CALENDAR DAYS.
1. PRIOR TO REMOVING ANY VEGETATION, CONTRACTOR SHALL MEET WITH THE PROPERTY OWNERS AND THE
PROJECT MANAGER TO REVIEW THE LIMITS OF CONSTRUCTION AND OBTAIN PERMISSION TO REMOVE
VEGETATION REQUIRED TO DO THE WORK.
2. TREE AND PLANT ROOTS OR BRANCHES THAT MAN INTERFERE WITH THE WORK SHALL BE TRIMMED OR
CUT ONLY WITH THE APPROVAL OF THE PROJECT' MANAGER. ANY TREES OR PLANTS WHICH ARE SHOWN
TO REMAIN THAT DO NOT INTERFERE WITH THE WORK, BUT ARE DAMAGED BY CONTRACTOR OR HIS
SUBCONTRACTORS, SHALL BE REPAIRED OR REPLACED BY CONTRACTOR AT NO ADDITIONAL COST.
3. CONTRACTOR SHALL CLEAN UP, RESTORE, SEED AND MAINTAIN ALL DISTURBED AREAS IMMEDIATELY
UPON COMPLETION OF WORK ON EACH SITE. TOPSOIL, SEED, FERTILIZER AND MULCH SHALL BE PLACED
IN ACCORDANCE WITH ESC AUTHORITY STANDARDS ON ALL DISTURBED AREAS. A PERMANENT STAND OF
GRASS ADEQUATE TO PREVENT EROSION SHALL BE ESTABLISHED PRIOR TO FINAL ACCEPTANCE.
4. THE ACT OF RESEEDING SHALL NOT QUALIFY AS 'STABILIZED". THE CONTRACTOR SHALL BE
RESPONSIBLE TO ESTABLISH VEGETATION, NOT SIWPLY APPLY SEED AND MATERIALS.

ANDERSON. THE SUBSTANTIVE CHANGES TO THE LAYOUT AREAS FOLLOWS:
