Dear Ms. Schweller,
At this time, your application has been deemed incomplete due to the deficiencies listed below. In
accordance with Section 5.1.40(e)(3)(d) of the Albemarle County Zoning Ordinance, the running of time
for action will be tolled until all required information is received.
[5.1.40(a)(4)(d), 5.1.40(b)(11)] Color. A paint color (Sherwin Williams Java Brown) was
provided for "the monopole, antennas, and related equipment" on Sheet A-2. Please clarify if the
ground equipment, ground equipment shelter, concrete pad, and/or fencing will be painted the
same color.
2. [5.1.40(a)(4)(e)] Topography. Please revise Sheet C-lA to include the full area within 2,000 feet
of the proposed facility. A review of this drawing showed that approximately 1,200 feet were
visible to the north and east of the proposed facility.
[5.1.40(a)(4)(g)] Setbacks. Please revise Sheet C-1 (or similar site plan drawing) to show the
proposed facility's setback distances from adjoining properties. The 75' fall radius over adjoining
properties is shown on this sheet, but not setback distances.
4. [5.1.40(a)(4)(i)] Location of certain structures and district boundaries. Please revise Sheet C-1
(or similar site plan drawing) to show the proposed facility's setback distances from adjoining
properties. The 75' fall radius is shown on this sheet, but not distances.

Comments reflect information available at the time the development proposal was reviewed, and should
not be considered final. However, the Site Review Committee has attempted to identify all issues that
will be required to be resolved prior to approval.

1. [5.1.40(c)(3)] Setbacks. The County Attorney will need to approve the proposed fall zone
easement documents as to form and style before this application can be approved: please provide
a draft copy of the easement deed language for the County Attorney's review. Please note that the
easement does not need to be recorded at this stage, but shall be recorded prior to issuance of the
building permit.
2. [5.1.40(b)(2)(c)] Projection. Please label the following distances on Sheet S-1:
• Distance from facility to closest and farthest points of the back of the microwave antenna.
• Distance from facility to closest and farthest points of the back of the Sector 2 antenna.

1. Building and electrical permits are required for the tower foundation, any structure greater than 150
sqft housing equipment, and the electrical service.
Albemarle County Service Authority (ACSA) — Richard Nelson
1. SDP201800043 Lewis Mountain Tier II PWSF will require RWSA approval. I have contacted
Victoria with RWSA and they are working with Milestone Communications on this.

Dear Mr. Knuppel:
The Department of Transportation, Charlottesville Residency Transportation and Land Use
Section, has reviewed the above referenced plan as submitted by Milestone Communications-
Shentel, dated June 04, 2018 and find it to be generally acceptable.
A VDOT Land Use Permit will be required prior to any work within the right-of-way. The
owner/developer must contact the VDOT Charlottesville Residency Land Use Section at (434)
422-9399 for information pertaining to this process. If further information is desired, please
