The submitted affidavit is not signed or notarized. 3. Section 14-400 – A private street will be required in order to provide access for each new lot onto State Route 620. Please re-label the easement as a “private street” and extend it from TMP 115-22G north to the new lot line proposed between TMP 115-22G1 and Parcel X. 4. Section 14-316 – The plat has been transmitted to VDOT for approval of the private street entrance and sight distance easement onto State Route 620. 5. Section 14-303(N) - Please provide the statement pertaining to private street standards and maintenance on the plat. 6. Section 14-317 – Please submit an updated private street maintenance agreement to replace the one recorded in D.B. 761, PG. 153. An editable template is attached. 7.
