County Code section 17-410 and Virginia Code §62.1-44.15:34 requires the VSMP authority to act on any
VSMP permit by issuing a project approval or denial. This project is denied, but is eligible for approval
pending print SWPPP submittal addressing minor comments, below, and completion of nutrient credit
purchase. Applicant may submit two print copies of rev. SWPPP, four (4) copies of full-size rev. VSMP
plan with letter stating `not for review, for approval' with implicit understanding nutrient credit purchase is
required for final stamped approval: (2.30 lb./yr.).
A. Stormwater Pollution Prevention Plan (SWPPP) — Approval pending print submittal with minor
revisions. (Ref. Applicant email, July 23, 2018 10:54 AM; County email, 7/30/2018 12:23 PM).
(Rev. 1) To be Addressed via print submittal (2 copies :
1. Sec. 415: Provide 8.5" X 11" ESC and SWM plan sheet inserts, once comments addressed.
2. Revise Sec. 9, Signed Certification to clarify Scott Collins is President of CE, not Owner of Creekside V,
Phase 1 & 2, and is not Owner as defined by ordinance NAC.
3. Sec. 6 —Include concrete wash -out (location) on both SWPPP exhibits.
SWM Plan — Approved, pending nutrient credit purchase
(Prior Engineering review comments addressed /acknowledged with Rev. 1)
Water Quality: Provide SW routing /calculation booklet. Ensure VaRRM.xls is included. .PDF of report is
welcome. If SW calculations were submitted with initial submittal, they became separated along the way.
Revised riser design:
a. Provide detail for debris cage /trash rack for the two (2) 4" diameter low flow orifices to ensure
these orifices function as designed, and are not obstructed.
b. Provide detail for solid steel plate or similar non -erosive cover on permanent low -flow (4" DIA)
orifices to avoid error, mishap, or misunderstanding during construction, and to ensure adequate
performance. Specify design, including are of curvature required to ensure watertight connection
between plate and riser.
3. Sheet 6
a. Intent of the two Post -Development Impervious Area Summary tables is not immediately apparent.
Clarify via text notes. Also, recommend call reviewer to discuss as aid to understanding. (Rev. 1)

Addressed: One table pertains to stormwater quantity (includes off -site drainage area), the other to
stormwater quality (restricted to area under developer's control /ownership).
b. Applicant should contact Management Analyst Ana Kilmer at 434.296-5832 —x3246 as first step in
acquiring Nutrient Credits, via purchase. Purchase without coordinating with Ana will likely delay
VSMP plan approval. (Rev. 1) Comment acknowledged. As follow-up, plans show 2.30 lb./yr.
nutrient credit purchase meets VRRM .xls total phosphorus load reduction requirement.
c. Provide deed book /page of Ex. SWM Facility easement that includes drainage channel from
SWM detention basin (approved with WPO201200062) to unnamed trib. to Lickinghole Creek.
d. Project proposes 0.79 Ac. be placed in (stom+water-) forest /open spaoe easement. Provide platted
easement and Deed dedicating 0.79 Ae. to peFmanent for-est/open space, per- SNAIM .
(Rev. 1) Addressed, design revised to eliminate reliance upon Forest /Open Space Easement.
2.30 lb.IX. nutrient credit purchase is required to meet state stormwater quality requirements.
B. Pollution Prevention Plan (PPP) — (Rev. 1) Pending: Review pending print submittal.
The PPP content requirements can be found in County Code section 17-404.
1. See A., above.
C. Erosion and Sediment Control Plan (ESCP) — Approved (Prior review comments addressed with Rev. 1
Virginia Code §62.1-44.15:55 requires the VESCP authority to approve or disapprove an ESCP. This plan
is disapproved for minor reason outlined in single comment, below. The erosion control plan content
requirements can be found in County Code section 17-402.
Sheets 4, 5:
a. CE (consistent with VESCH): This is fine. In addition, please provide labels or notes requiring
paved construction entrance. (Ref. detail, sheet 5).
Process
After approval, plans will need to be bonded. The bonding process is begun by submitting a bond estimate request
form and fee to the Department of Community Development. One of the plan reviewers will prepare estimates and
check parcel and easement information based on the approved plans. The County's Management Analyst will
prepare bond agreement forms, which will need to be completed by the owner and submitted along with cash,
certificates or sureties for the amounts specified. The agreements will need to be approved and signed by the
County Attorney and County Engineer. This may take 2-4 weeks to obtain all the correct signatures and forms.
An existing Stormwater Management Facilities Maintenance agreement appears to cover this development, but
County's Management Analyst may require documents to verify that Owner (Old Trail Creekside V, LLC) is
eligible to discharge storm runoff to a SWM Facility dedicated to Old Trail Community Association, Inc. (BK
4626/PG 574). Anticipate Management Analyst will perform review needed to establish that existing SWM
Facilities Maintenance Agreement is sufficient.
After bonding and agreements are complete, county staff will need to enter project information in a DEQ
database for state application processing. DEQ will review the application information based on local VSMP
authority approval. At this time, the DEQ portion of the application fees will need to be paid directly to the state.
For fastest processing, this is done electronically with the emails provided on the application. DEQ should notify
applicants with instructions on how to pay fees. When DEQ approves the application, they will issue a permit
coverage letter. This should be copied to the county.
Area of Disturbance is listed in VSMP /WPO plan as 6.71 Ac.
After DEQ coverage is issued, via the coverage letter, the County can hold a pre -construction conference.
Applicants will need to complete the request for a pre -construction conference form, and pay the remainder of the

application fee. The form identifies the contractor and responsible land disturbed, and the fee remaining to be
paid. This will be checked by county staff, and upon approval, a pre -construction conference will be scheduled
with the County inspector. At the pre -construction conference, should everything proceed satisfactorily, a joint
VSMP and grading permit will be issued by the County so that work may begin.
