RE: Old Trail Village Blocks 10, 16, 17, 18 and 30 As -built Road Plans
I have made an inspection and performed as -built surveys of the roads in Old Trail Village
Blocks 10, 16, 17, 18 and 30. I certify that the roads have been built in accordance with the
approved construction plans.
As -built public road plans dated January 25, 2016, and last revised on July 15, 2015 were
prepared based on field surveys of the final constructed improvement locations. The plans have
been submitted to the Virginia Department of Transportation and County of Albemarle. The as -
built plans show both the final horizontal and vertical alignments for the roads, the locations and
elevations of storm drainage pipes, the locations of all stormwater conveyance channels and
easements and all public road signage. All horizontal and vertical variations between the
approved plans and the as -built features are indicated on the as -built plans.

Mr. Rector,
The asphalt material placed at Old Trail has been produced per the current VDOT approved mix
design. SLWCO designs and produces all of mix per these mix designs unless specific project
specifications are given to us that describe a different material blend.

This form is intended to facilitate the completion or bonded improvements in a timely manner by assuring the
developer has a schedule in place, and by providing this Department a means to assess the developer's ability to
meet the completion date stated in the bond agreement. The developer must provide a completion date for each
milestone listed below.

Submit all paperwork in accordance with the County's "Public Road Acceptance Procedure"
(certificate of completion, as -built plans, recorded plats, professional engineer certi fication
for completed improvements, etc.).
VDOT documentation of acceptance (request to Board of Supervisors to adopt
resolution to accept road(s) into secondary road system).
Private Roads
Start of construction.
Submit al I paperwork in accordance with County's "Private Road Acceptance Procedure"
(compiled report of CBR tests, stone depth inspections, and fill compaction tests,
certified by a professional engineer, certificate of completion, as -built plans, recorded plats,
professional engineer certification for completed improvements, etc.).
Request 45 -day inspection from County.

Water & Sewer Improvements
Improvements dedicated and "acceptance" letter issued by the Albemarle County Service
Authority.
Additional Bonded Subdivision Improvements flandscaning, pedestrian oaths, sidewalks etc.
Submit signed and sealed letter from a professional engincer listing and certifying that
completed improvements are built according to approved plans and request inspection
from County.

By signing below, the owner/developer agrees to complete the bonded improvements in accordance
with the above schedule. if, during the course of construction, the owner/developer finds they will
be unable to meet the schedule, they must provide written justification to the County Engineer of
the reasons preventing completion along with a revised completion schedule.
