0 ❑ ❑ Construction activities are not substantially disrupting the movement of aquatic life.
0 ❑ ❑ Proper E&S controls are present, properly maintained, and functioning adequately.
0 ❑ ❑ Stormwater measures are present and properly preventing stormwater runoff.
0 ❑ ❑ Surface waters temporarily affected by construction activities have been restored to
original elevations and appropriately stabilized.
❑ ❑ 0 Utility line work in surface waters was performed to minimize disturbance to surface
waters and the area was returned to original contours and appropriately stabilized.
❑ ❑ 0 In -stream work is being performed in the dry with the appropriate use of cofferdams
sheetpiling etc, to minimize disturbance of the stream bottom and increases in turbidity.
0 ❑ ❑ Pipes and/or culverts for road crossings are countersunk a minimum of six inches to
provide for the re-establishment of a natural stream bottom and low flow channel.
❑ ❑ 0 Wetlands outside the construction areas are clearly marked to prevent additional
unpermitted wetland impacts and no additional wetland impacts have occurred.
D ❑ ❑ Stream banks have been adequately stabilized.
❑ ❑ 0 Time -of -year restrictions regarding impacts to surface waters are being adhered to.
0 ❑ ❑ No additional impacts to surface waters have occurred.
GENERAL COMMENTS:
DEQ staff met with Mr. Jess Achenbach for the site inspection to observe site conditions prior to processing a
termination request for the permit. DEQ staff observed conditions at the main Carrol Creek crossing and a small
impact in the residential section for the installation of a stormwater management BMP and outfall. Both impacts
appear to be taken as described in the JPA.
GENERAL NON-COMPLIANCE ITEMS NOTED:
Erosion and sediment controls are still present at the Carrol Creek crossing. DEQ staff requested that all E&SC be
removed and the entire site stabilized prior to processing termination of the permit. Mr. Achenbach indicated
that he will check with the County and remove the E&SC if they agree site is adequately stabilized. Mr. Achenbach
indicated that he would provide photos when complete.
