General Notes:
❑ Stabilize all reconstructed,repaired,and bare areas within the right-of-way with fertilizer,seed and mulch.
❑ Sweep the road of loose stones and debris prior to final inspection.

O Reviewed A video camera inspection is required for all storm sewers and culverts that are deemed inaccessible to VDOT
inspections and are to be accepted into VDOT's maintenance program and ownership.The video inspection
shall be conducted in accordance with VDOT's video camera inspection procedure.

AS-BUILTS and PLATS to be reviewed for easements,road centerline,grade to see if other problems are

These items will be updated on punch list and resent after review for correction if needed.
AS-Builts are Required to show existing grades,planned grades and finished grades of roadway along with drainage items
and easements.
1) Backfill and Stabilize all areas seed and straw behind the sidewalk and curb and gutter.
2)Straigten all stop signs that are loose and leaning.Tops should be cut to have 16"above the top of post only.
3)Clean and Sweep the road and curb and gutter. Flush all DI throats.
4)Curb and Gutter, Sidewalk will be re-inspected on areas of road where house construction is taking
place for damage.
5)See Attached Video Report that has been reviewed with clean out needed.
6) Need Quit Claims from Utility companies in right of way.

Street: Misc. Items ALL STREETS
1)All stop signs will need to be 30"with 16"above the top of stop sign for VDOT route markers to be placed and 7'from
the bottom of stop sign to the ground for pedesrtian head clearence.
2)All speciality signs,plantings, irrigation equipment that is in the right of way will need to have a maintneance agreement
with a permit to keep in place and will need to be completed after the road officially is in the state system.
3)Other problems that may exist or have occurred since thie review that is not on this 45 day punch list will be added.
Metal Post for State Signs will have a 7 gauge base and 10 gauge post.Wood post 4x4 can be used as well.
4)County 911 Signs are to be located to the left of the intersection on there own post.

3)All Roller Patterns and Density Test for Sm-9.5A.And BM-25.0 Have.

6)All CD and UD types and sizes will be required prior to final overlay to have a video camera inspection to check for problem
GENERAL NOTES: All asIhalt re•airs will be milled and saved at the existins "rade of 'fanned roadwa desk n.An se-
ction not marked during this review does not relieve the developer of the responsibility of other places that have settled or
intersections that have been overlayed above the designed grade of the remaining sections of roadway that have not been
disturbed for open cuts and overlayed with out being milled and paved property. Developer will review and call if futher
review of a section or spot in the field is needed from VDOT to be marked between the final inspection.
Notes:
All test reports for Subgrade Test, Asphalt and Stone density and roller patterns and structure backfill test, fill
reports.CHECK AND GRADE ALL AREAS AND MAKE SURE THEY WILL DRAIN PROPERLY.
STABILIZATION MATTING OR RIP RAP SHALL BE USED TO STABILIZE WHEN NEEDED.
ALL AREAS SHALL BE STABILIZED WITH GRASS GROWING BEFORE FINAL ACCEPTANCE.

RE: Old Trail Village Blocks 1B & 3C As-Built Road Plan
I have made an inspection and performed asbuilt surveys of the roads in Old Trail Village Blocks
1B & 3C. I certify that the roads have been built in accordance with the approved construction
plans. Asbuilt public road plans dated June 9, 2015 and last revised on March 14, 2017 were
prepared based on field surveys of the final constructed improvement locations. The plans have
been submitted to the Virginia Department of Transportation and County of Albemarle. The
asbuilt plans show both the final horizontal and vertical alignments for the roads, the locations
and elevations of storm drainage pipes, the locations of all stormwater conveyance channels and
easements and all public road signage. All horizontal and vertical variations between the
approved plans and the asbuilt features are indicated on the asbuilt plans.

stormwater facilities will require construction record drawings, inspection reports,and completion processes
through VDOT or DEQ. Bonds will not be reduced or released without record drawings and inspection
documentation per the state requirements and county's published procedures. A minimum 20%of the original
posting is held until acceptance of all bonded improvements. For Site Development Performance bonds, if
landscape installation is part of the bonded incomplete site work,a request for reduction should only be made
once all site work is complete.
