Inspections for the purposes of bond reduction or release will require a separate fee for each bond.
Submit requests to: Department of Community Development,401 McIntire Road,North Wing,Charlottesville,
VA 22902,Attention: Management Analyst-Phone 434-296-5832; Fax 434-972-4126. All roads and
stormwater facilities will require construction record drawings, inspection reports,and completion processes
through VDOT or DEQ. Bonds will not be reduced or released without record drawings and inspection
documentation per the state requirements and county's published procedures. A minimum 20%of the original
posting is held until acceptance of all bonded improvements. For Site Development Performance bonds,if
landscape installation is part of the bonded incomplete site work,a request for reduction should only be made
once all site work is complete.

General Notes:
❑ Stabilize all reconstructed,repaired,and bare areas within the right-of-way with fertilizer,seed and mulch.
❑ Sweep the road of loose stones and debris prior to final inspection.
❑ All post larger than 4"x 4"shall be drilled with a hole configuration equivalent to VDOT standard 602.3
o Mailboxes are to be 18"from the edge of pavement to the post.

❑ Grade all shoulders and ditch lines to the approved plan cross section.

❑ See List A video camera inspection is required for all storm sewers and culverts that are deemed inaccessible to VDOT
inspections and are to be accepted into VDOT's maintenance program and ownership,The video inspection
shall be conducted in accordance with VDOT's video camera inspection procedure.

❑ UUdate Two copies of the recorded plat and as-Built plans 8.5 inches by 11 inches.

Street: Claremont Lane
AS-BUILTS and PLATS to be reviewed for easements,road centerline,grade to see if other problems are
in need of adjustment.Will be reviewed and comments provided if there are adjustments to be made.
These items will be updated on punch list and resent after review for correction if needed.
AS-Builts are Required to show existing grades,planned grades and finished grades of roadway along with drainage items
and easements.
1)Final overlay in place.Any damage that happens will need to be repaired from house
construction.Damage will be repaired in half lane widths and 10'on either side.
2)Currently there are no damages to the curb and gutter and sidewalk that is in place.
Repairs will be required if needed from damage during house construction.
3)Mark right of way line and put drainage easement deed book and page number on as-built.
4)Storm Pipe that needs to be cleaned out and new video: Structures 1-1,5-3,7-5
7-9 and 18A-22.These have mud,rock and other trash stopping up water.
5) General Cleanup, backfill,seeding and straw behind curb and gutter and sidewalk areas.
Street: Rowcross Street

1)All stop signs will need to be 30"with 16"above the top of stop sign for VDOT route markers to be placed and 7'from
the bottom of stop sign to the ground for pedesrtian head clearence.
2)Alt speciality signs,plantings, irrigation equipment that is in the right of way will need to have a maintneance agreement
with a permit to keep in place and will need to be completed after the road officially is in the state system.
3)Other problems that may exist or have occurred since thie review that is not on this 45 day punch list will be added.
Metal Post for State Signs will have a 7 gauge base and 10 gauge post.Wood post 4x4 can be used as well.
4)County 911 Signs are to be located to the left of the intersection on there own post.
Street: Mandatory Testing Needed

2)All Roller Patterns and Density Test for#21-A or B Stone. Have
3)Ail Roller Patterns and Density Test for Sm-9.5A.And BM-25.0 Have

6)All CD and UD types and sizes will be required prior to final overlay to have a video camera inspection to check for problem
GENERAL NOTES: All asphalt repairs will be milled and paved at the existing grade of planned roadway design.Any se-
ction not marked during this review does not relieve the developer of the responsibility of other places that have settled or
intersections that have been overlayed above the designed grade of the remaining sections of roadway that have not been
disturbed for open cuts and overlayed with out being milled and paved properly.Developer will review and call if futher
review of a section or spot in the field is needed from VDOT to be marked between the final inspection.
Notes:
All test reports for Subgrade Test, Asphalt and Stone density and roller patterns and structure backfill test,fill
reports.CHECK AND GRADE ALL AREAS AND MAKE SURE THEY WILL DRAIN PROPERLY.
STABILIZATION MATTING OR RIP RAP SHALL BE USED TO STABILIZE WHEN NEEDED.
ALL AREAS SHALL BE STABILIZED WITH GRASS GROWING BEFORE FINAL ACCEPTANCE.
All CBR reports for all roads with a asphalt design attached will need to be sent in as well prior to paving.

RE: Old Trail Village Block 15 As-built Road Plans
I have made an inspection and performed as-built surveys of the roads in Old Trail Village Block
15. I certify that the roads have been built in accordance with the approved construction plans.
As-built public road plans dated May 9, 2014, and last revised on September 26, 2016 were
prepared based on field surveys of the final constructed improvement locations. The plans have
been submitted to the Virginia Department of Transportation and County of Albemarle. The as-
built plans show both the final horizontal and vertical alignments for the roads, the locations and
elevations of storm drainage pipes, the locations of all stormwater conveyance channels and
easements and all public road signage. All horizontal and vertical variations between the
approved plans and the as-built features are indicated on the as-built plans.
