This letter is to notify you that the following development proposal has been submitted to the County for
review:

LOCATION: 299 Colonnade Drive(Rivanna Water& Sewer Authority Lewis Mountain water tank
site)
PROPOSAL:Request for approval of a Tier II personal wireless service facility with a 72.5'tall steel
monopole located within a 477 square-foot fenced equipment compound area. The site will be
accessed using an existing 25' access road and utility easement.
ZONING: R-15 Residential- 15 units/acre

COMPREHENSIVE PLAN:Urban Density Residential—residential(6.01-34 units/acre);supporting
uses such as places of worship,schools,public and institutional uses,neighborhood scale commercial,
office,and service uses;and Parks and Green Systems—parks, playgrounds,play fields,greenways,
trails, paths, recreational facilities and equipment, plazas, outdoor sitting areas, natural areas,
preservation of stream buffers, floodplains and steep slopes adjacent to rivers and streams. In

This development proposal is scheduled for the Site Review Committee meeting on the date listed below.
The applicant's attendance is not mandatory, although it may be requested by the County. Regardless of
whether the applicant will be attending, the County staff will be available for consultation on the Site
Review Committee meeting date at 10:00 a.m., in Room 235 of the County Office Building on McIntire
Road in Charlottesville. This meeting will be an informal session with the Site Review Committee to
discuss issues and share information.
Prior to the Site Review Committee meeting,the Site Review Committee will review the development
proposal for compliance with the applicable ordinances or other applicable laws. At the Site Review
Committee meeting, on the date noted below, one of two things will happen, either;
1. Staff will provide a recommendation of approval stating the requirements that must be included

with submittal of the final site plan and those conditions which must satisfied prior to approval
of the final site plan and, where applicable,those conditions which must be satisfied prior to
issuance of a grading permit.
OR
2. Staff will provide a recommendation of disapproval stating the reasons for disapproval by
identifying the plan's deficiencies and citing the applicable sections of the Zoning Ordinance or
other applicable laws, and what corrections or modifications will permit approval of the plan.
Within fifteen(15)days of the disapproval of the initial site plan the developer will be able to
resubmit a revised plan.

Development. To assure the file information is ready for your viewing and to allow staffto be available to help

do give priority to those with appointments. Those without appointments may find the materials are in use by

Re: Balloon Test for Milestone Communications Tier II Wireless Personal Wireless Service
Facility ("Lewis Mountain Water Tank")
Dear Neighbor:
Milestone Communications invites you to attend a balloon test on Wednesday, June 27,
2018 to be conducted during the hours of 9:30 a.m. — 11:00 a.m. at the address of 299
Colonnade Drive, Charlottesville, Virginia 22903, property of Rivanna Water and Service
Authority, with current Albemarle Tax Map parcel number 07600-00-00-002A1. In the case of
inclement weather, the test may be canceled and rescheduled for Thursday, June 28th, at the
same time and place. If the weather is questionable, please call me at 804-248-8700 for a status
update.
Milestone Communications has submitted an application for a Tier II Personal Wireless
Service Facility consisting of a 72.5' monopole with a 2' foot lightning rod, flush-mounted
antennas and related ground equipment. The purpose of a balloon test is to evaluate the potential
visual impact of the proposed facility by raising to the height of the proposed monopole a large
(4'-5') red balloon and viewing it from neighboring roadways and properties. Representatives of
the applicant and the County will be onsite at the beginning of the test and will be off-site much
of the time to evaluate the visibility of the balloon and take photographs for photosimulations.
